今年9月,美国海岸警卫队(USCG)在其最新的船舶滞留名单中扣留了两艘散货船、一艘油轮和一艘多用途支持船,其中包括一艘主机无法在驾驶台或轮机控制室远程操控的船舶。

9月29日,美国海岸警卫队安检员在休斯敦扣留了73,400载重吨的成品油船“Seaworld***”号,原因是该船存在多项安全缺陷。

在这些缺陷中,该机构指出该船的主机动力操控方面存在问题,PSC检查官检查发现,该轮驾驶台和集控室操远程控主机功能失效。

美国海岸警卫队表示,船上船员称,自 6 月份以来,他们一直采用机旁(应急)操作方式操控主机运行。“该情况已经上报给公司,但没有得到有效修复”。

缺陷还包括船舶设备的维护、机舱油料管理和电气方面等问题。

缺陷:

1. 缺陷代码:15109 - Maintenance of the ship and equipment船舶和设备的维护

公司应制定程序,确保船舶维护符合相关规则和条例的规定以及公司可能制定的任何其他要求。

在满足这些要求时,公司应确保:.2 报告所有不符合项的情况及其可能的原因;.3 采取适当的纠正措施。

.2 any non-conformity is reported with its possible cause, if known;.3 appropriate corrective action is taken.

02-08 缺陷的技术和/或操作性质提供了严重违反《ISM规则》的客观证据。船员已向公司报告情况,但以下情况并未向船级/船旗报告,也未得到纠正。

The technical and/or operational nature of deficiencies 02 - 08 provide objective evidence of a serious failure of the implementation of the ISM Code. The crew is reporting conditions to the company, however the below conditions are not being reported to class/flag nor are they being rectified.

在消除此缺陷之前,必须提交一份令OCMI满意的外部审计报告。

An external audit report satisfactory to the OCMI must be submitted prior to clearance of this deficiency.

2. 代码:13101 - Propulsion main engine 推进主机

对船舶推进和安全至关重要的主要和辅助机械应配备有效的操作和控制手段。PSCO从驾驶台和轮机控制室观察到主机无法操作。船员称他们自6月份以来一直从当地(应急)平台操作主机。该情况已上报公司,但未得到纠正。

Main and auxiliary machinery essential for the propulsion and safety of the ship shall be provided with effective means for its operation and control. PSCO observed the main engine is inoperable from the bridge and engine control room. The crew states they have been operating the main engine from the local (emergency) platform since June. This condition has been reported to company without rectification.

3、07126 - Oil accumulation in engine room 轮机舱积油

本规定的目的是防止点燃可燃材料或易燃液体。为此,应满足以下功能要求:

The purpose of this regulation is to prevent the ignition of combustible materials or flammable liquids. For this purpose, the following functional requirements shall be met:

● 必须提供控制易燃液体泄漏的手段 (means shall be provided to control leaks of flammable liquids);

PSCO观察到以下地方有漏油和积油现象:

    - 主机辅助鼓风机周围、所有五个燃油泵、所有五个气缸盖、曲轴箱防爆盖3 - 5,以及主机前方的底舱。

    PSCO observed oil leaks and accumulated oil: -around the auxiliary blower, all five fuel pumps, all five cylinder heads, crankcase explosion covers 3 - 5, and in the bilge forward of the main engine.

    - 1号和3号发电机飞轮周围。3号发电机的飞轮周围有一个收集漏油的特制接油口。-应急发电机油箱周围。

    Around the flywheel for generator 1 and 3. Generator 3 had a fabricated catch collecting oil leaks.

No electrical equipment shall be installed in any space where flammable mixtures are liable to collect including those on board tankers or in compartments assigned principally to accumulator batteries in paint lockers, acetylene stores or similar spaces, unless the administration is satisfied that such equipment is of a type which will not ignite the mixture concerned. PSCO observed an explosion proof light located in the battery room on the navigation bridge missing a glad in one of the stuffing tubes. PSCO observed 2 explosion proof lights located in the cargo deck store, which is indicated as a hazardous location per the vessels approved hazardous location plan, retro-fitted with improper glands.

9月20日,一艘76,000载重吨的干散货轮“M***”在哥伦比亚河被扣留,原因是该船在安全和环境政策方面存在缺陷。

The Company should establish procedures to ensure that the ship is maintained in accordance with the provisions of the relevant rules and regulations established by the company. The technical and operational nature of the deficiencies noted below provide objective evidence of a serious failure of the implementation of the ISM Code. An external audit report satisfactory to the Coast Guard must be submitted prior to clearance of this deficiency.

9月24日,海岸警卫队官员在洛杉矶扣留了37,500载重吨散货船 “CS ***”,原因是该船存在多项缺陷。

其中包括在检查官发现有证据表明该船未能在完全执行《ISM规则》后对船舶和设备进行维护。

其他缺陷包括轮机舱积污油水,以及船上救援艇的问题。

缺陷:

1、15109 - Maintenance of the ship and equipment船舶和设备的维护

THE VESSEL DID NOT TAKE APPOPRIATE ACTIONS ON IDENTIFIED NONCONFORMITIES. THIS IS MADE EVIDENT BY DEFICIENCIES 2-7. A SAFETY MANAGEMENT AUDIT MUST BE CARRIED OUT BY THE ADMINISTRATION / RO BEFORE THE SHIP MAY BE RELEASED FROM DETENTION.

应提供控制易燃液体泄漏的手段。PSCO发现该船的两台发电机和主柴油机周围有过量的燃油浸泡。

MEANS SHALL BE PROVIDED TO CONTROL LEAKS OF FLAMMABLE LIQUIDS. PSCO OBSERVED EXCESSIVE FUEL OIL-SOAKED LAGGING AROUND TWO OF THE VESSEL’S GENERATORS AND MAIN DIESEL ENGINE.

3、07126 - Oil accumulation in engine room轮机舱积油

应提供控制易燃液体泄漏的手段。PSCO发现该船的两台发电机上积聚了过量的燃油。

MEANS SHALL BE PROVIDED TO CONTROL LEAKS OF FLAMMABLE LIQUIDS. PSCO OBSERVED AN EXCESSIVE AMOUNT OF FUEL OIL ACCUMULATION ON TWO OF THE VESSEL’S GENERATORS.

4、14119 - Oil and oily mixtures from machinery spaces

应配备有效的舱底抽水系统,能够从任何水密隔舱抽水和排水。PSCO观察到,由于附近的压载泵每分钟泄漏约一加仑的油水,发动机舱舱底井内积聚了约六英寸的油水。

AN EFFICIENT BILGE PUMPING SYSTEM SHALL BE PROVIDED, CAPABLE OF PUMPING FROM AND DRAINING ANY WATERTIGHT COMPARTMENT. PSCO OBSERVED AN ACCUMULATION OF APPROXIMATELY SIX INCHES OF OILY WATER IN ENGINE ROOM BILGE WELLS, CAUSED BY AN APPROXIMATELY GALLON A MINUTE LEAK FROM NEARBY BALLAST PUMP.

5、11105 - 救援艇清单

所有救生设备和安排均应符合本规范的适用要求。除另有规定外,每艘救援船的正常设备应包括一盏探照灯。

ALL LIFE-SAVING APPLIANCES AND ARRANGMENTS SHALL COMPLY WITH THE APPLICABLE REQUIREMENTS OF THE CODE. EXCEPT AS OTHERWISE STATED, THE NORMAL EQUIPMENT OF EVERY RESCUE BOAT SHALL CONSIST OF A SEARCHLIGHT. 

PSCO发现救援船的配电盘安装/固定不当,导致探照灯、罗盘灯和便携式照明灯的插座无法按设计运行。

PSCO OBSERVED THAT THE ELECTRICAL PANEL FOR THE RESCUE BOAT WAS IMPROPERLY MOUNTED/SECURED CAUSING THE SEARCHLIGHT, COMPASS LIGHT, AND RECEPTACLE FOR THE PORTABLE LIGHT TO NOT OPERATE AS DESIGNED.

9月4日,2,623载重吨多用途支持船“Subsea*** ” 号于在得克萨斯州阿瑟港因应急发电机和船上操作问题被扣留。

美国海岸警卫队表示:该公司应制定程序,确保船舶维护符合相关规则和条例的规定。

该船船员无法应要求提供SMS程序,也不熟悉该船的安全管理系统。基于这一点以及缺陷的技术和操作性质......提供了严重违反ISM法规的客观证据。

缺陷:

1. 代码:04114 - 应急电源 - 应急发电机 Emergency source of power - Emergency generato

可用电力应足以在紧急情况下提供对安全至关重要的所有服务,同时应适当考虑到可能需要同时运行的服务。轮机长称2号气缸有缸套漏水,气缸套可能有裂缝。为避免发动机可能发生故障,他并未启动发动机。

The electrical power available shall be sufficient to supply all those services that are essential for safety in an emergency, due regard being paid to such services as may have to be operated simultaneously. Vessel's Chief Engineer stated there was a jacket water leak on the number 2 cylinder, a possible crack in the cylinder liner. Did not want to start the engine to avoid a possible engine failure.

2、15106 - 船上作业Shipboard operations

该公司应制定程序,确保船舶维护符合相关规章制度的规定。该船船员无法应要求提供SMS程序,也不熟悉该船的安全管理系统。基于此,以及以下指出的缺陷的技术和操作性质,提供了严重违反ISM规则的客观证据。在该船离港之前,RO(ABS)需要进行一次额外的外部核查。

The company should establish procedures to ensure the ship is maintained in conformity with the provisions of the relevant rules and regulations. The vessel’s crew was unable to provide SMS procedures upon request and were unfamiliar with the vessel’s Safety Management System. Based on this and the technical and operational nature of the deficiencies noted below provide objective evidence of a serious failure of implementation of the ISM code. An additional external verification is required to be carried out by the RO (ABS) prior to the vessel’s departure from port.
